Acceptance of Terms

By accessing or using ClaimRevive at claimrevive.netlify.app (the "Service"), you agree to be bound by these Terms of Service. If you do not agree, do not use the Service.

Description of Service

ClaimRevive is an AI-powered tool that analyzes medical claim denial information and generates appeal letters. The Service uses Anthropic's Claude API to process the data you provide and return a drafted appeal letter and denial analysis.

User Responsibilities

You are solely responsible for the accuracy of all information you enter into ClaimRevive, including patient data, claim details, clinical notes, and denial codes. You are responsible for reviewing and editing all generated appeal letters before sending them to any insurance payer. You are responsible for ensuring compliance with all applicable laws and regulations, including HIPAA, when handling patient information. You acknowledge that ClaimRevive generates appeal letters as a drafting aid and that the final content must be reviewed by a qualified billing professional or healthcare provider before submission.

No Legal or Medical Advice

ClaimRevive does not provide legal advice, medical advice, or billing consultation. The appeal letters generated by the Service are AI-drafted documents intended as a starting point. ClaimRevive makes no guarantee that any appeal will be successful or that payers will overturn any denial based on content generated by the Service.

No Guarantee of Results

While ClaimRevive is designed to assist with denial appeal letter generation, we do not guarantee any specific outcome, including but not limited to: appeal approval, claim payment, revenue recovery, or payer response. Results depend on many factors outside our control, including payer policies, clinical documentation quality, and denial circumstances.

Intellectual Property

The appeal letters generated by ClaimRevive are yours to use, edit, and distribute as you see fit. The ClaimRevive name, logo, and underlying technology remain the property of ClaimRevive.

Limitation of Liability

To the maximum extent permitted by law, ClaimRevive and its operators shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of revenue, data, or business opportunities, arising from your use of the Service. Our total liability for any claim arising from or related to the Service shall not exceed the amount you paid for the Service in the 12 months preceding the claim.

Availability
